- What was measured and why.
- Source system and reporting period.
- Inclusion and exclusion criteria.
- Numerator, denominator, and missing data.
- Whether the analysis is prospective or retrospective.
- Known limitations and conflicts.
- Version and review date.
Evidence informs education without replacing case judgment
Cases and cohorts help learners recognize patterns, ask better questions, and understand why the system evolved. They do not guarantee that the same outcome will occur in every cat, facility, team, or setting.
